Dead on arrival?

My new USBUIRT doesn't seem to be functioning. I had some intermittent functionality from the Beyond TV setup wizard, but for the most part, the USBUIRT has been unresponsive. I've even had hard crashes while trying to run that portion of the setup wizard. The system would reboot whenever it tried to communicate with the USBUIRT. FWIW, this is a system that has been flawlessly stable, running 24x7 as a HTPC for over a year. I'm trying to upgrade from an ATI setup to a dual-tuner PVR-500 setup. If the USBUIRT is connected, the system becomes unstable.

In the course of troubleshooting, I was able to do a flash upgrade once, but now get consistent "Unable to connect" messages. Just to be sure that it wasn't a problem with my USB hub, I tried a direct connection. Still no good. Then I tried installing the USBUIRT drivers on two laptops and plugged the USBUIRT into each. None of these systems would show the USBUIRT in Device Manager, and all display the same error message from the Flash utility.

I'm convinced that this one is dead right out of the box. I purchased this on 22 March through Snapstream. What must I do to get a replacement?
